OBJECTIVES OF A CBC SYSTEM
The objectives of a CBC (Community-Based Care) system include but are not limited to the following:
Discharge Planning
at Entry
ensures that children and families entering the child welfare system are promptly assessed and provided with a comprehensive plan for safe and timely reunification or alternative permanent placement options.
Involving
Juvenile Justice
involvement in community-based care will provide families with faster adjudication and prevent trauma as much as possible and provide children with a safe environment.
Behavioral Health Resources & Services
ensures children and families have timely access to comprehensive behavioral health interventions, including mental health and substance abuse services, as well as preventative supports to address underlying issues and mitigate potential harm.
Normalize
Community Placements
prioritizes placing children in stable and supportive family environments within their communities, such as with kinship caregivers or foster families, to minimize disruptions and promote their overall well-being
Less Reliance on Residential Placement
reduces the use of expensive & disruptive institutional care, such as group homes or residential treatment centers, and instead focuses on providing appropriate therapeutic interventions and support services in community-based settings.
Flexible Case Management
is to provide individualized and flexible casework services that adapt to the specific needs and circumstances of families, enabling them to access the necessary resources and supports to achieve safety, permanency, and well-being.
Earlier Preventative
Services
plays a crucial role in keeping families united and out of foster care by addressing potential challenges and providing the necessary support before they escalate.
Utilization of
Kinship Care
engages and involves extended family members and relatives as potential placement resources for children, recognizing the importance of maintaining their connections and cultural identity.
Community Support
for Reunified Families
to collaborate with community partners to provide ongoing support and resources to families who have been reunified, ensuring their successful transition and preventing future involvement with the child welfare system.
